Consultation & Consent forms.

all consent and consultation forms are filled out electronically through vagaro. these forms must be filled out and submitted before your appointment at mint skin. if you have any questions about filling out these forms, please feel free to email me @ mintskin.waxskincare@gmail.com or send a text!

When booking your appointment, the appropriate consent form should automatically pop up before booking your appointment. if you are a regular client and have had any changes at your next appointment that may effect your service, please let me know. i.e.; if you are taking any new medications/stopped medications, if you are pregnant/nursing, allergies. these are important changes to make sure you are protected during wax, skincare, and lash services!

*NO SHOW/LATE CANCELLATION POLICY: TO AVOID ANY FEE’S, IF YOU HAVE TO RESCHEDULE YOUR APPOINTMENT FOR ANY REASON, YOU MUST RESCHEDULE WITH AT LEAST 24 HOURS NOTICE. Due to a busy schedule, I cannot afford to run behind; IF it has been 10 minutes or more and you are not checked in for your appointment, your appointment will be cancelled and a late cancellation/no show fee will be charged; THE AMOUNT OF EACH CANCELLATION IS SPECIFIC TO THE COST OF THE SERVICE(S) originally scheduled and then MISSED OR CANCELLED WITH less than 24 hours notice.

*IF YOU NO SHOW/CANCEL WITH LESS THAN 24 HOURS two or more times, YOUR ONLINE BOOKING ABILITY WILL BE RESTRICTED TO REQUIRE ACCEPTANCE by myself BEFORE BOOKING YOUR APPOINTMENT.

*If there is an unpaid balance after 5 days, your vagaro account will be disabled and unable to book appointments.
